Application Data Sheet

The HealthVault Application Information form is used to gather information about the specific application to be reviewed as part of the Go-Live process. HealthVault will use the Application ID in this document to verify that your application configuration complies with the guidelines set forth in our Go-Live Guide. Other items in this document are used by our marketing team in order to find additional opportunities to introduce relevant HealthVault applications to other projects.

Please copy and paste the information specified in HealthVault Application Information page into a Microsoft Word document and send the completed document to the HealthVault Go-Live Team when you ask to start the Go-Live process.

Once we receive all required information we will review it and schedule a one-hour block of time for you to do a live demo of your application. Your application will be reviewed based on the data types usage data flow between your application and HealthVault and use of HealthVault branding elements. The review will be used to provide feedback regarding your application, including possible required fixes before your application can Go-Live as well as some recommendations for vNext of your application. The entire process from receipt of the Go-Live request to the migration of the application to the production environment typically takes 2-4 weeks. This is dependent upon how quickly we are able to schedule the review meeting and the time for all required changes to be made.
